Please call, email or complete the form below if you have any questions or would like to schedule an appointment. When calling to schedule an appointment as a new patient, you will be asked the following:
- If you are scheduling for a routine check up and cleaning or if you have a specific problem.
- Days/time of day that is best or most convenient for you to schedule an appointment.
- What dental insurance you have, if any. In order for us to contact your insurance and get a break down of benefits, we will need the name of the insurance company, the subscriber name, ID number and date of birth.
- Your name, address and contact numbers.
- You will be informed of our 24-hour cancellation policy.
